 Patient Forms

Our new patient forms will need to be completed prior to you seeing the Doctor. 

Our forms are available for you to download from this site, in the New Patient Centre and they provide us with information on your condition and a history of the physical, mental and chemical stress that may be affecting your nervous tissue and other core body systems. 

Upon entering our office, our receptionist will welcome you as a member of our family, simply turn in your forms and your appointment will begin shortly.


 Consult and Insight Scan

The Doctor will meet with you briefly to determine, based on your patient forms what neurological scanning procedures are to be performed.  These test will document the extent of the effects of stress and tension on the nervous tissue layer.  Because of the sensitive nature of these tests, they need to be completed prior to a full examination.


Full History and Consultation

Next, you will have a full consultation with the Doctor to discuss all of your health-related problems, concerns, and potential treatment options. This allows the Doctor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.


Complete Examination

After your consultation the Doctor will perform a complete chiropractic examination including standard neurological, orthopedic, postural, and physical tests.  These tests will show where and how stress and tension are affecting the muscle and joint layers of your body.  Please note, nothing will be done in our office  without your consent.


X-Ray Studies

Depending on your specific condition, the Doctor may order X-rays.  X-rays help us develop  the most effective treatment plan for you, and alert us of  any serious spinal conditions.  X-rays are covered by OHIP, but they will need to be taken at a local x-ray lab - in most cases X-rays can be obtained the same day.  Once you have a copy of your x-rays in hand- drop them off so that the Doctor may evaluate them.


Once all the relevant information has been gathered we can then schedule a time for the doctor to give you a full report on the results of your tests.


 2nd Visit:

Doctor Report of Findings

The information we gathered from the initial visit will allow the doctor to put together your individual plan to guide you through the phases of care: Release, Rebuild and Wellness.  Your plan will incorporate the necessary services and/or products to meet your unique needs.

Your detailed report of all findings will answer all of your questions including:

1. How can you help me?

2. How often do I need to come in?

3. What  will  my treatment  cost?

Feel free to ask questions at any time.  Remember, everything we have incorporated at the centre, whether it is additional therapies to nutritional products to custom orthotics, is designed to support the release of stress and tension, rebuild your health and provide you with the tools to be healthy.

Chiropractic Assistant Report

Once the Doctor has answered all your questions an office assistant will help you make the scheduling of your care convenient and easy.  They will also make you aware of special pay plans available to maximize any insurance coverage you may have and to make your care as affordable as possible.


Treatment

 Following the Doctor's and the Assistant's report, you will experience your first treatment. This will include stress relieving spinal adjustments.  This will mark the beginning of the Release phase of your care.  Treatments are designed to release built up stress and tension.  Treatments are also interactive, so you can express concerns about the different styles of treatment anytime.
